For current and new patients, please contact our office by phone to schedule your appointment. Emergency calls and appointments are available when needed. You can schedule online too via this link!

New Patient Forms
For our new patients, please complete our paperwork in advance by clicking this link.

Financial
We are happy to submit all insurance forms for you and help you recover the most from your benefits. We do everything in our power to help you afford the treatment you need. Forms of payment we accept are check, cash, and major credit cards.

• Our credit card processing system has a 4% service charge.
• To avoid this fee, payments by cash or check.

Cancellation
If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. We ask for this advance notice so that we can offer the appointment to another patient. A fee may be charged if a patient does not show up for an appointment without sufficient notice.

Reminder!
Chiropractors are entry level physicians and seeing one does not require a referral from a medical doctor!
